Getting Started

Whether you’ve been referred to us by your physician, a trusted friend or family member or possibly just word of mouth, you’ve made a step in the right direction.  Your initial consultation is an important step in the process.  Spend some time researching the procedure you’re considering and form questions to discuss with Dr. Smith and our staff.  By coming prepared, you’ll get the most out of your visit.

What to Bring 

On the day of your first appointment, we ask that you bring your insurance card and a form of identification (ID). Your primary doctor may ask you to bring additional material. If a patient is under the age of 18, they must be accompanied by a parent or guardian. The first appointment typically lasts one hour, so make sure to clear your schedule to allow yourself enough time to ask questions. 

Making an Appointment 

To schedule your first visit, call our office at 517-908-3040. Our friendly staff will find a time that fits with your busy schedule and do our best answer any questions you have. To cancel an appointment, please give us a call 24 hours prior to your visit. For specific questions or concerns, refer to our contact page and one of our staff members will be in touch shortly.
